I think it's a way of sublimating and making up for areas they're not religious in. For example, the father of the kid who's bris I was at lets his kids' watch TV on Shabbat which is a big no-no.

The circumcision itself is what needs to be done. Jewish law even explicitly states that even if Metzitzah is not done then it is still a valid circumcision. The Talmud itself just says metzitzah, nothing about the mouth, that comes later when Jewish law gets codified.
