I see nothing historically inaccurate about it. Not sure if it is based in a primary source. I assume John Adams would have met him like this at some point as the diplomat to Great Britain.

He was a pretty good king, but he did suffer from some pretty severe illnesses particularly starting around the time of the Revolution and particularly after the Napoleonic Wars. He practically got deposed at the end of his reign. Not his fault though; he was a product of inbreeding.
Honestly feel like everything after this episode downhill. The rest of the series basically just tells how Adams descends into a crotchety old fart who is always in other people's shadow.
