There are no real successors for a chancellor in Germany. There's a vice chancellor, but he's only supposed to represent the chancellor now and then, he won't get to succeed the leader (unlike in USA), but you're kinda right. The Bundespräsident will take over, but only to enforce the Bundestag to vote for a temporary chancellor until the next elections come around.
Her private physician (who should know best about what's going on) didn't leak anything yet, and I don't give much about other doctors trying to diagnose her by watching a video.
