Looking for Jow Forumsack programmer

Need help with making form based database website.
Main idea is:
Visitor of the website fills form consisting of 3 pieces of data, one of which is a drop-down.
I should be able to filter that information. E.g. If a drop down is "apple, orange, pear," then I should be able to see only "pears." etc.
I am willing to pay ~30$ in crypto. If it's too difficult, willing to pay more. Need help with putting it on a website too.

dude what you're wanting consists of about 30 minutes of minimal php/css/html work. what you are not saying is how much time will be added when you want to change the layout 50 times, add superdupercool graphics and set up ssl and a site icon and blahblahblah shit. you want a real website, we can contract.
