I don't think you'll see any fighting in the Midwest or in New England. Hell, I'd be surprised if any fighting occurs in the Southeast. You make get some small skirmishes here and there but nothing too serious that the US government will be able to lock down. Furthermore, once the Southwest and West Coast are lost, whatever secessionist movements do popup elsewhere are more likely to be peaceful. All in all, I think most of the conflict will be relegated to the Southwest and everything west of the Rockies.

The longer is takes for it all to occur, the more violent the conflict will be and the more likely Canada will get to enjoy the conflict as well.
