It would quickly spiral into trying to take hold of cities/installations and the causalities would be huge. Probably the other world powers would get involved either directly or indirectly and there would be a real chance of world war 3 happening. Relatively early one side would gain big advantage over the other and we would enter a long guerilla war style war where citizens would actually be involved. Theres less than a 0.01 % chance of this ever happening.
