Humans take at least a decade to become reliably fertile and a breeding program would take dozens of generations to make significant progress. A program like this would take at least a century and probably two or three. You also take the risk of introducing more problems than you solve. e.g. a higher intelligence population but they all have crippling social anxiety or a heart condition that kills them by the time they're 20 years old.

That depends entirely on what sort of traits are unwanted. This also runs into the problem where an unwanted trait may be blocking out an even worse trait from surfacing. We have no way of determining why certain functions of the body evolved to do what they do or what sort of pressures they experience. Sickle cell anemia, for example, is an obviously inferior circulatory system compared to most human subjects, but it also prevents death from malaria. If you're stuck in sub-Saharan Africa, not dying from malaria is worth having the inferior red blood cells.

Basically, a human breeding program would be guesswork at best. You might get a away with a population of less than 1000 or you might end up needing a million people. Human genetic diversity is already pretty small thanks to some genetic chokepoint that occurred thousands of years ago, so the risk of running into horrific health issues is pretty high.
