If your kid is not born on base then you have to apply for citizenship. Way back in the day I knew some people who were based in Iran. The wife was pregnant with their first. Then the Shah was in the process of "falling" and Americans were being evacuated.

The wife went into labor and due to conditions was not able to make it to the American hospital and had the kid in Iran. They got evacuation orders like two days later not having time to get the baby registered American or any paperwork.

Iranians were checking all the Americans leaving via a checkpoint for papers. So the wife buries the baby (carefully) in a bag and rolled that Iranian citizen right through the checkpoint and onto the plane. They legit said it was the scariest thing they ever had to do.

The wife had to take a taxi to the Iranian hospital and describe how the taxi drivers had a picture of the Shah and a picture of Khomeni in their taxis and would post whichever picture they needed to each day to get through the crowds unmolested.

The military kids have to be born in military hospitals to be considered automatic US citizens and get papers.

A relative married a German man and had their son there. She was able to register him at the embassy. Big mistake to have the kid in Germany and even bigger mistake to be married to a German financial criminal (worth millions). The Germans nailed the husband and stuck his ass in prison and she could not leave Germany with the kid to come home unless he signed the papers. So she was stuck living in Germanistan on the dole until she convinced him to sign the paper that she was only "coming for a visit". Actually I know a few people like that. Once you have another kid (nonmilitary) in a foreign country you are basically screwed if the spouse won't sign. And being born in another country does NOT give citizenship automatically. You have to file for it and do all kinds of paperwork.
