There's tests for all of these things, but the definition of what constitutes ADD/ADHD and bi-polar or schizophrenia is basically up for debate.

It has less to do with "lower dopamine", but the level of response of certain neurons (e.g. D1, D2) to certain amines. The responses can be permanently modulated by c-FOS induced genetic changes. Variations in the FOS gene have been verified between races and individuals through oncology research, which means you basically have no clue what you're talking about.

There's tons of people who are lazy, and a majority are probably misdiagnosed or use it as an excuse, but literally the same pathways affects addiction, depression, and a number of other things which have real-world tests, including brain scans, with actual scientific data to back it up.

From basic undergrad biology, dopamine is converted into norepinephrine, which binds to your beta-adrenergic receptors. These pathways, for example, directly controls how much energy uptake can be performed by your muscles. This is exactly the reason why people with Parkinson's have a lack of fine motor control.
