I don't think much would change. There wasn't such a taboo around nuclear weapons that there is today during the second world war, I think that mindset is something that arose out of generations being brought up to believe that nukes would kill them at any second. During WWII it was generally assumed that whoever got the bomb would use it in combat and this obviously ended up being the case. Call it a necessary evil, in reality or your scenario the message being shown by nuking Japan is that nukes being used on an actual enemy population isn't just some hypothetical concept, it's a very real possibility. I think that influenced both sides in the cold war as other nations got the bomb and arsenals grew.

Iirc X-day planned on using nukes on the landing beaches. So a shot ton of american's with cancer. I think the interesting part comes if the US bombs Japanese cities with the nukes but the japanese don't surrender. It is likely that the Soviets would have eventually landed their own forces on the home islands if they saw a chance to grab more of east Asia which would have led to another north/south like in Korea and Vietnam. It is also likely that Japanese civilian losses would be astronomical do to their intense fanatisim and devotion to the emperor who, in this scenario, declared no surrender.

I think we would have used the other 5 on Russia and there would be no cold war. I'm pretty sure a reason why we used the nukes in the first place was to stop the war fast enough that Russia wouldn't start a massive offense on Manchuria and Korea, expanding their reach (I think I read that in Truman) . I think we probably would have been aggressive at the time and nuked Moscow and all time major Russian industrial centers.
