It might help if you open a map of MD. That is a single district that has been shaped to divide republican voters in central MD so that the dems will hold the majority and have more reps in the house of reps.
The right angle under "Montgomery (county)" is DC. The right side is the Chesapeake Bay. To the north, just off the map is the Pennsylvania line. So as you can see, the district loops and weaves around the state in a very unnatural way to make sure there are no republican strongholds by mixing them with heavily democrat areas (like inner city Baltimore).
There's a saying here: "In most states, the voters choose their representatives. In Maryland, the representatives choose their voters."
