Stronger breach typically means heavier. Hotter loads means more recoil. Pistol cartridges between .380 and 10mm are at the optimum for the current technology and use cases.

Self defense is the only scenario which is understandable to not have ear protection, and I think the one time occurrence shouldn’t be too terrible

Velocity and sectional density defeat armor, not bullet weight or energy. While .17 and .20 caliber guns have some problems, they could allow similar penetration to 5.56 from a pistol size cartridge. With a mag in grip design you can add several inches more barrel, which helps counter lost velocity from the smaller caliber.
