S9 many require even a new bolt or barrel to prevent stuff like this from happening I would doubt any company would make "realistic" looking training rounds that could also fire from an unmodified gun. Simunition bolts will feed a live round. The whole point of the system is that it's built around the same cartridge dimensions and magazine as the rifle before conversion.
Swapping the bolt is so the weapon still cycles with the reduced gas from the FX rounds. It's not a safety feature.

FX version of pistols are an exception. They require more extensive modification of the weapon, so can incorporate additional safety features.

No one is immune to fuck ups in training. There was a story a SEAL wrote about in a book (a SEAL writing a book, gee what a novel concept). According to the story a group of SEALs were getting ready to assault a building on a rainy day. There was an exterior set of stairs that the team was in the process of ascending. One of the team members in the stack slipped on the wet stairs, inadvertently discharging a round and striking his team mate in the back, which he ended up killing him. There's another story outta Iraq about a dude from 10th SFG that accidentally burned down an entire kill house when he accidentally went into the kill house with some incendiary rounds loaded. Apparently a fire resulted that caused a total loss of the structure. Point being no one is immune from fuck ups regardless of what unit they are from.
