This is just my experience, but if you're hitting 15 with 40kg you should at least be able to hit 12 with 45kg fairly easily. If that doesn't work then stick to 40kg doing 15 reps for a session or two, THEN move to 45kg. You shouldn't have a problem with that. Also remember to eat enough and sleep.

Form is always the most important factor imo. If you can't execute the exercise properly due to too much weight then you don't increase the weight of course. If you can't get 12 reps with that weight you can also lower the weight and do more reps to get a good pump.

Just make sure you also vary with different exercises.
