I'm very dedicated to self-improvement and I live a very austere life, I don't know if this is "monk mode" but I do need tons of self-discipline and it didn't come easy because I was a total procrastinating failure a few years ago.

So given that, let me give you some advice, because you know it comes from someone with similar goals and problems as you:

Don't do nofap. Total abstinence will do more harm than good. Reduce it to 1x/week because you get a proven test spike, and if you're struggling with self-discipline you can use it as reward.

Don't quit porn. If you're stuck in some endless porn-masturbation cycle porn can be bad, but if you just watch it, it can be good. Studies prove it raises test and helps your lifts if you watch it before a workout.

I would not quit meds. If you were told to take them that means you have a condition and in order to function properly on daily tasks, you need them. Facing this condition soberly, coupled with abstinence from meds, will add a lot of chaos through your daily routine and derail the whole project. Leave the abandonment of medicine for a period where you have your habits and routine figured out and embedded into your subconscious, that's what I did with antidepressants.

Don't try to quit everything/start doing everything at once. That approaches tend to fail. If I were you, I'd break every goal into steps. If you want to, say, eat well, you can make step one quitting sugar and crap food, step two eating more protein, step three taking supplements, step four counting calories, step five educating yourself on how to eat for better gains, step six implementing that, etc. Each week you take a step. You should use some spreadsheet program to make a table of those, and give yourself goals for at least the next couple of months. But make sure no step (or combination of weekly steps) is too overwhelming. You can use the same method for gradually decreasing the consumption of whatever you can't go cold turkey with.
