He probably has some amount of social anxiety similar to me. In my case I've never managed to initiate conversations myself, so I've only made friends from other people starting conversations with me. In addition it's a lot harder to meet people after high-school where you often don't have the same classes with the same people anymore.
Most of the friends I've made have been through existing friends, since being around a good friend makes it much easier to be relaxed and social.
One way you could help if you live near him is help him meet new people with him, so that way he'll be more comfortable/social.
I've moved to another country recently so I can't help him like that, but you're probably right about the things you said. He has "friends" from his HS years, but no new ones.
