My anxious hypochondria aside, is visualizing really an indicator of anything? I assume people blind from birth have limited visualizing ability, likewise for the deaf and internal "voice", and yet none of the blind or deaf people (including my own father) exhibit and serious cognitive deficits.

Whether or not you are an NPC really shouldn't be about an internal monologue or ability to visualize things. It's about the ability to make plans and execute then despite the world and your sporadic feelings changing. It's about the ability to understand yourself and the world well enough to predict the results of your actions, then using that knowledge to make your life and the world around you the way you want it to be.

An NPC is someone who falls into predictable cycles and patterns that don't move their life forward or help them in any way.

I remember seeing someone here on r9k use the NPC term over a year ago and I thought it was great. That was before it became political.

I still like it though. I tend to have a lot of social anxiety, but I find that if I just think of the people I'm interacting with as NPCs in a videogame it makes it easier for me to deal with them. If you still control your life, use all the drugs you want. I smoke weed, but I moderate myself and still control my life. But even still I can feel the pull off habit that makes you want to take the day off and sit around stoned. Falling into addiction, always succumbing to the urge, and losing control of your life makes you a predictable NPC. It might have some particularly sad or dramatic dialogue options, about all the things it used to have in it's life, but it's an NPC if the patterns control you instead of you controlling the patterns in your life.

Even the player is non entirely free. They are free to interpret, enjoy or dislike the story as much as they want. Players progress through the story differently, it might even make the story different. But to some degree, the story will always be the same. The spawn point is set and the end credits don't change, no matter what avenue you pursue throughout the story. I have heard of some people glitching and breaking the game though; going out of bounds and seeing things the devs didn't intend them to see.
