The fourth ending is unlocked through the choices you make throughout the story. It's the only ending locked behind a points system. It's the alternative to the Tower end.
You can do that at the same time as reading. Just try the no hands mode.

It's not as if it's random. There's a certain mindset you have to adopt picking the choices. And it certainly isn't a hard requirement (maybe 60%). Try picking the opposite of what you picked on the Tower route.
