You cannot force compliance with treatments. People break their diets and refuse to lose weight for their surgeries all the time. At a certain point you need to accept that some people will refuse to take any personal responsibility for their health. Carbohydrate(sugar) is fat storing, is not used directly for energy and lowers TEE(total energy expenditure output). Further because carbs crank on insulin, they drive on blood sugar creating an energy imbalance that leaves the body in a state of energy deprivation, thus lacking satiety. Its why people who eat sugary foods are constantly hungry, have to fight the physiologically increased urge to eat more.

The problem is that people have become complacent with eating trash 24/7 and not thinking anything of it. You can probably walk to the nearest McDicks if you don't live in bumfuck nowhere. The difference between people now and people during and before WW2 was the availability of fast food places, the cost of healthy eating choices and the perception of being healthy vs. being fat. Rice is a carb and is consumed it vast quantity in asia which also has the lowest obesity rates. If you eat more energy than you need in a day, the excess is stored as fat. This is regardless of source(fat, protein or carbs). The answer is in education of your TDEE, restrictions on the size of plates and bowels that can be sold and portion size limits at all restaurants and fast food outlets.

Like, from my own experience, I dropped quite a lot of weight with minimal effort when I was a poor college student because I couldn't afford snacks. I didn't necessarily eat great meals -- pizza was a common order -- but I rarely snacked.
In the past year or so, I struggled to lose weight despite being a lot more conscious about eating right and properly exercising. Biggest difference? A LOT more snacking (including reasonably healthy things like rice cakes), even when combined with lower meal caloric intake.
The main issue is that people believe that the benefit (a technically true benefit, mind you) of being able to think very clearly and be on top of things due to increased blood sugar is worth any downsides that it might lead to (even worse -- most people are 99% of the time not aware of any downsides besides calories). So, whenever they get minor brain fog and lethargy due to going without food, they say "man, I should get a snack"; or, perhapsthe same thing but add in a stomach growl: "man, my body is telling me to eat, so I should!" And it's easier than ever to sate those cravings: we have bars, shakes, etc., all of which should help you out but often don't.
It sucks to say it, but people have to learn to live with being a little hungry. Because LITERALLY 99.99999999999+% of your ancestors spent nearly all of their day *not* eating. Even your relatively recent ancestors likely only ate a couple times a day. And they were fine: they birthed the modern world.
This is correct but nobody in first world countries today has the balls to support that. Even when you draw the clear legal distinction between voluntary and involuntary medical risk factors it won’t be favored because it “feels” bad. And ultimately that feeling is all that matters to the public in modern politics.
