i just go until i can only do one more, and then stop. don't bother counting usually unless i'm going for hypertrophy, and even then the specifics don't really matter all that much.
I do sixth reps just to be sure all the time.

I draw an imaginary rectangle with 5 points on each side under the bench. I do 10×10 so i put my keys or something on the points and move them one step around the rectangle after each set. I never lose count of what set im on now.
I have such shitty memory that I actually note down in my phone a number every time I do a set. Otherwise I'd do 6th sets all the time. Reps it's pretty rare but still happens

I do something similar when I do abs. I have to hold this position for a few seconds and I always lose count of the rep I'm on so I look at the bolts of nearby pec deck
I work out at the library. Now I can count to 10 without issue.

I tend to not care about reps as in general my rep range falls into....
3-5 (overload)
5-8 (strength training)
8-15 (working)
15-20 (for reps)
+20 (endurance training)
The exact reps dont matter and can vary based on rest period and a handful of variable...so being over/under by 2 reps or so....who cares...
But I tend to lose track of set # for some reason. I have myfitnesspal but it slows me down having to log into it and generally get distracted by phone when I pull it out.
Thought about bringing in a paper/pen for tally marks, but that seems like a pain.

For sets I increase my rests by 15-30 seconds every time, starting at 1:45 and ending at 2:30 on 5x5s, or for 3x5s I do 2:00 and then 2:30. The hard part is just remembering I’m only doing 3 sets of a certain exercise after coming off a 5x5. With reps I’m pretty much always though
