For the passable FtM ones, you can tell that they are at least gay or that something is off. (i know, i hosted one and a few friends for a night and I could tell something was off, it's only in the morning that I realized the lack of adam's apple and the different name on its credit card)
For the MtF it's really a dichotomy, but without makeup you can absolutely tell.
