They are certainly hard to get used to. Takes a week or more if you're used to playing videogames. If not, it takes a day to adjust, albeit slow

To avoid wrist cramps they should be, I doubt they are better than a regular mouse though.

This is a cheap vertical mouse by Anker, the one in OP's pic is the Evoluent and is like three times more expensive but totally worth it.

I bought this IXCC vertical mouse super cheap, like $10. I think it's more of a diagonal mouse to be frank, so there's not a lot of retraining to do. My only gripe is there seems to be the tiniest bit of lag (maybe all wireless suffers from this?) so it's not for gaming, but it's fine for general office work. It has absolutely saved my wrist at work, and maybe staved off carpal tunnel for another few years.

It's cheap, but ergonomically it's the same. It's weird to use. Raising your thumb for back buttons. Get tiring. Also your scroll finger orientation changes. It's weird, and I can't get used to it. My hands are normally sized.

It's important to have the proper angle though.
Lots of cheap ones don't have a aggressive enough angle to relieve stress on your rotary cuffs/wrists/tendons

It's not the most functional for gaming but for that I use a gamepad.
For work though I use a vertical mouse.

I was seriously considering getting one for work because I found myself elevating one side of my mouse pad to emulate the feeling of one to ward off wrist pain, but then I saw the light and started using the keyboard for most things (i3, vimperator), and the trackpoint for most pointy stuff. Now I only use a mouse for the occasional video gaming session, seldom enough that I'm fine using a regular mouse. I'm a cheap fuck and feel bad spending 50 € on a mouse, otherwise I'd buy one. Maybe when my current mouse breaks.

I use a vertical for work and a regular for computing at home. Keeps my wrists from dying and is pretty easy to use. I would not recommend verticals for gaming, but it's not impossible to do so. Like others have said there is a learning curve.

Thing is the diagonal mouse actually decreased strain on my wrist considerably. It is indeed ergonomic, just perhaps not as therapeutic as a full blown vertical mouse. Each according to their own need user.

I just use a trackball. Works fine.
