As someone who studies this, higher end viruses and malware are often "gangs" or groups that do this for the money involved. C2 operations and shit like that. Theres a handful of groups that do things like Adware and malvertisements. Theres some whitepapers out on things like Poison Ivy and its growth and development into different tools.

Russians, Chinese and Americans are almost always the actors.
