The problem is that the Android permissions have fairly broad over arching requests.
Say I have an app that plays audio in the background and I want it to check if you are in a call to reduce the volume or do some user defined event.
The permission I would request is READ_PHONE_STATE, the permission prompt that the user would see is the same as in OP's pic.

This is a case where being up front about why you're asking for certain permissions and being open source are good things, because what the user sees could appear way more malicious than what is actually being requested.

You can get a 2nd layer of permision with lineageos privacy guard, and even trick apps that demand permissions that you have given them
