I bet you used custom cables from a previous power supply on this new build with a new psu or cheap ones you got off ebay. and didn't realize that even thou all modular cables use same plug on PSU end they send dif volts on dif rails and you need dif wired ones for each of the 8 PSU brands.

vram dead/failure, had same thing happen to my gtx 670, same sort of lines, had to get a new gpu. You'll have to RMA the card. Might be an unfortunate manufacturing defect with the card itself, for it to have such an issue that suddenly into its lifetime (my gtx 670 was about 5 and a half years old)
