Not me but a friends experience.
He very briefly worked as a sort of sysadmin for a local company. Everything ran as root. Everyone had full admin privileges in AD. Some Windows licenses were pirated. Every server was fully exposed to the internet, SSH on port 22 with password auth, remote desktop exposed to the internet. So on and so forth. As far as he told me they did everything you learn your first semester at uni not to do ever under any circumstances.
They are still operating and probably didn't fix anything.

i’m new so i’m not sure, but a common complaint is our corporate proxy. it’s a fucking mess and set up poorly, blocks a lot of development and progress more than it prevents people watching Netflix.

No offense but that's a super rare problem (if the reboot was the source of it) and if the settings were super important you should have said so. Not having backups is not entirely your fault, however.

We don't have a ground wire in the whole building. Well, it's there in the sockets but it's not connected to anything. We had one the power plugs burn a little due to sparking and the live wire was shorted with the ground wire. Our network rack became a live wire and so did every computer. 20 people were working that.

IT guy lost a almost all the hardware dongles for critical software during a move. Manage to fudge one into working. Software vendors have now switched to gay "cloud" licensing.
Wish opensource CAM wasn't so ass, otherwise I'd just take it as an opportunity to switch to tool chains that aren't constantly ass fucking you.

Accidentally ran an incorrect database insert on the memo table for 600,000 records. Our database is a multivalue db, not SQL, so removing it was pretty much impossible, except manually, one by one. I ended up applying another insert saying "the previous note was entered in error, please disregard," but people were not happy my garbage was in every almost every account in our system.

This happened like 20 years ago. The details aren't all that clear now but think this was it roughly:

RAID array. Flash tech at the time. Monkey wanted to show us all how you could hotswap the drives. So he pulled out one of the disks live, lets call slot A. Replaces it with a blank disk and the array starts rebuilding. Goes to swap it back but accidentally pulls out the disk in slot B. Puts old slot A into slot B. RAID array crashes, we go down for days scrambling to recover from tape. A few days of data that wasn't committed to tape was lost, including some business critical information. Monkey was fired.

System was completely unresponsive had to shutdown as OOM process killer couldn't even deal with it at that point.

back when i was 10, i fucked around with the computer at home disassembling it and shit. naturally not knowing shit it wouldn't boot, so my dad yelled at me and we took it to the store. i told the fuckers no format, but they did of course. they formatted the device first, after realizing it still wouldn't boot THEN they opened the panel to check: turns out i didn't seat the ram properly

DNS was fubar'd on every single computer, they had to put the entire network on lockdown to prevent it from spreading to the child companies' networks. Now every usb drive goes under tight scrutiny before being allowed (they liked my work enough to move to one of the child companies as junior sysadmin)

Tutorial is supposed to guide user through setting up a dummy version of the companies work that the new employee can fuck around with.

However, the tutorial's examples included directions that made new employees fucking around actually affect the REAL company work instead of the DUMMY work.

The new employee wasn't paying careful attention, the company had very stupid instructions, and so the guy lost his job and the company lost all its work.

It's standard procedure for some lazy fucks to just rip all the harddrives out and drop a master sysprep image onto them for fast deployment.
IT at the school I worked at did that to all the hundreds of computers in the multiple labs once an year to "clean off the gunk and update shit".

Lots of computer repair shops I've been to have some sort of custom image from shit like Norton fucking Ghost that they load on to everything that gets brought in.

No one knows who or why. All they could do was track it down to the source pc, which is used by 30+ people (it was from one of the production lines' machinery). Now every usb drive is whitelisted at hardware id level only after we manually check it.
Well it was back in 2014, but yeah, those pc's had no protection at all, which was worsened by the machine's software requiring an admin login to function (yes, running it as admin isn't enough).

Well, in our organisation, it's because it's not their job to do anything else.

I'm a SOE developer. It's my job to make a working WDS/MDT image that works across the enterprise.

If a machine isn't working, it's almost always a hardware failure. Our contract with the OEM requires we ensure it isn't a software fault. So we have to try reimage first. We are aware it is often futile. After that'd been tried, we call out the OEM and they replace the device.

If it was unseated RAM - We don't care. That's out of our responsibility and it's on the OEM to come out and fix it. That's how large enterprises do it.

If it's a software issue like a bad driver, it's going to be affecting more than one machine because the same driver is pushed out across the entire enterprise and we've only got a couple different models.

It's not the IT helpdesk person's job to work out the problem. It's my problem to make sure the drivers work. I'll get alerted to problems because a large number of servicenow incidents will be open for the same thing.

Some people just don't get division of responsibilities. Helpdesk people are not there to do everything.

for some reason my company gives everyone in software engineering the root password so everyone is always logged in as root. Much fuckery has occurred. to name a few, once someone shutdown a sever by trying to run: "ls -halt" but the connection was shitty and all the sever saw was "halt". whoops. another one was the office fuck up/retard that somehow keeps fucking up but they won't fire , accidentally ran " mv / /someotherdirectory/oof" and all of root was moved somewhere else...the client was told the drive failed and they bought it afaik. another time someone truncated the wrong SQL table making the server useless since backups we're disabled...again "mysterious drive failure".
