The only interesting malware I've ever seen was something that launched itself from explorer.exe's startup location in registry, and used special characters so unless you looked at the hex values of the key you couldn't see anything else was there. It was pretty weird and fun to clean. I don't remember what it was actually trying to do. Probably just adware.
Everything else has just been garbage that started itself from the same reg location as normal applications with varying levels of removal prevention. Most of them don't even need safemode to stomp.
