Yes, by embedding an image with a unique code in the URL. Most email providers proxy the image to prevent stuff like this though.

This is a very old technique but I think it was patched out yeaars ago.
I've got the same shit before. They're the real password, but they get them from leaked databases. In January, nearly a billion email/passwords were released onto the internet from various sites. It came from that and others like it.

He got the password the same way he got the email, from a public dump of someone's leaked DB.
Since emails are usually logins these days, he can just email all those logins and say hey, your password is XXX, I know because I hacked you.
He literally copied it out of some pastebin that someone dumped from X website.
