It's definitely not "only possible with UWP." There are plenty of Win32 programs with custom titlebars. You're posting from one. It would be pretty easy to make a libmpv frontend on Windows that looked like IINA, but nobody's done it yet.

Yes, if you only play 1080p videos. If you also plan to play 720p footage, you should test what luma upscaler you like most (FSRCNNX only doubles the resolution and the result of SSSR depends on the base scaler used). profile=gpu-hq sets spline36 by default. You might want to test other popular alternatives like ewa_lanczossharp. You could also look into chroma scalers, but the benefits are rather small. KrigBilateral is probably too resource heavy for your situation, but again, you can try the different in-built scalers and see if you have any particular preferences.

Yes, You could have checked out the links in OP and see that mpv is more customizable and has just has just as good shaders. It's more lightweight too. I can run FSRCNNX_x2_8-0-4-1.glsl on my laptop.

It's definitely heavily customizable. As for which one provides better looking output, you won't get an objective answer, because a) there's a lot of personal preference involved when it comes to video playback and b) most people are heavily biased towards their own player of choice, so they'd always defend it.
One thing I can say for sure though, madVR is better when it comes to HDR.
