There is some serious poverty due to systemic exploitation of the people by the outside coal companies, who come in and basically jew the fuck out of the locals and export all the profits. It's also where the stereotypical American hick comes from.
I believe that most people are nice though. I lived about 45 min away (Pittsburgh) so I spent a lot of time in WV. Western PA is mostly the same, except the mountains are smaller and it's not impoverished.
