Russia doesn't really need US permission to annex Ukraine. They simply chose not to. No one wants to police a large country full of angry locals. It's a huge money sink, and Russia isn't exactly that wealthy. Crimea is an important naval base for the Russian Navy and the government of Ukraine offered to build a NATO base right next to it That was a big no-no for the Kremlin. It threatened Russian interests in the region pretty badly. The Donbass area is the industrial center of Ukraine. Without it, the country is worth much less for the EU and right now Ukrainian Armed Forces are unable to secure it, thus potential Ukrainian ascension to the EU is next to impossible. Kaliningrad is also an important warm water port for the Russian Fleet and a major rocket base for their nukes. It will never be German. It's too important for Russia.

There is no American bases and troops in Ukraine and the USA has serious problems to even project power in Poland, not to mention such remote region like Ukraine connected to Russia in all possible ways : through military to economical ones and closed with Black Sea which is completely controlled by Russian navy. This little strip of land can be very easily and fast terminated in case of war anyway. Its strategic importance was radically decreased after fall of Soviet Union, it was useful only as long as Poland and Eastern Germany were a part of Soviet block. Today it can be neutralized in minutes even by Polish artillery so everything Russians placed there is supposed to have a political meaning rather than military one. Iskanders can easily reach territory of Poland from Western Russia after all

Find me artillery shells capable of reliably penetrating bunkers in which nuclear missiles are housed. Nukes fired from the Kaliningrad Oblast are able to bypass the American missile shield and that's a great asset all by itself. When push comes to shove those missiles can strike Warsaw, Berlin, and all the other major cities in Central and Eastern Europe practically unopposed. Moreover, it's fucking cold in Saint Petersburg and the harbor in Kaliningrad is warm all year. The Baltic Sea doesn't freeze here, but it freezes from time to time up north.

Not going to happen. War in Donbass was purposefully designed to be unwinnable by both sides from the get go, and neither the Ukrainian nor the Russian oligarchs want to end it anyway. It's too profitable for them. Additionally, it's a great scapegoat for all the internal problems those two countries have. It's a frozen conflict that will never be resolved as long as the geopolitical situation in the world remains the same. People like Poroshenko and Putin are rubbing their hands in glee when Ukrainian and Russian boys high on propaganda are shooting each other in Donbass.
