The question is not of cost, because having a single payer medical system is much, much cheaper than what we currently do.

Two things can cut that down: single payer insurance or more realistically for the USA, automation/AI.

It's not really an economic problem but a political one. Democrats would be the only ones who would bounce that ball into court, but Democrats have been corporatist cucks sucking big pharma's dick for like 2-3 decades by now. There is currently a progressive wing within the Democratic party demanding medicare for all, which might be the defining issue for their platform in 2020.
