There are places in America with no sidewalks, this is especially common in all kinds of suburbia. Their officials don't even expect anyone to walk, ride a bicycle or do any other kind or physical exercise. Because Americans hardly do any of these.
Add to that extremely unhealthy diets, consisting largely highly-processed ready food (barely anyone cooks in home anymore). That food is full of various nasty ingredients, including but not limited to HFCS and chemicals, because American food regulations are incredibly lenient compared to Europe.
But the worst part, aside from the lack of excercise, is the insane culture of eating. Somehow the companies managed to persuade people that more is better, that eating bigger portions is almost a patriotic duty and that drinking diet coke somehow makes your huge meal less fattening.
