It's a pun. The prison is called Shawshank, being "shanked" means being stabbed in prison, shaw = sure. So Shawshank = you're surely going to be shanked/stabbed.

not really, some do, some don't, usually they do it when they should find a substitute for a wordplay, sometimes to make a title more appealing or less silly/rude sounding which sometimes happens with translations
if they don't have a specific reason they leave it unchanged
btw the aforementioned пoдвoднaя бpaтвa originally shark tale wasn't translated aкyлья иcтopия because russian language doesn't have the idiom fish tale, and since that wordplay isn't a key one to the story they didn't try to substitute it with a similar one in russian, which also wouldn't be easy and simply used some stylistically fitting name
