I would guess, without really knowing anything, that Moscow and St Petersburg are first world lite (meaning barely first world) and the rest of the country is second world.

It is the literal definition of 2nd world, not that this outdated categorization makes any sense now.

those are cold war terms

1st world means allies to the USA
2nd world means allies to the USSR (Russia)
3rd world means unaligned

2nd world tends not be used anymore since the soviets fell, and 1st / 3rd world are attempting to fill the void
but really you're talking about developed / developing. which also isn't very good, since city by city / region by region in any country has noticeable differences
