Yes, it means you're literally unable to do anything else.
It used to not be so bad. About a generation ago, the fast food industry was filled with better workers. However, the pay has not kept up with demand, so the only people who will work there are people with no other options. I'm talking druggies and deadbeats. Service is terrible at most fast food places nowadays, to the point where I generally avoid them because I don't want to wait half an hour for a burger that looks like it was dipped in grease and thrown against the wall, and half the time they can't even get the order correct.
So yes, at the very least working somewhere like McDonald's carries a stigma, a kind of bad reputation.
