In the case of Portugal, AIDS became rampant because of heroin addicts sharing needles. The rates of new infections dropped when we changed drug which, besides making treating addiction simpler, makes it easier to collect used needles to dispose of and to provide sterile needles to people who are going to shoot up wether they have clean needles or only dirty needles.

In the case of former USSR, specially Russia, it had to do with belief that AIDS was a "capitalist disease" and low standards of care for hospitals (except for the more restricted hospitals meant for higher-ups). When AIDS first got to Russia, it spread like wildfire through it's hospitals where they reused all manner tools and authorities tried to downplay the severity of the epidemic since it was a disease "of prostitutes, homosexuals, homeless" and all sorts of things associated with western decadence.
