The study is made for fictional expats (expatistan), cost of living is correct in this context as this is what the company that is sending you has to compensate for.

but 'high' and 'little' are randomly chosen based on that blue line so one would imply you'd need to be above it. but if you remove it, you can shift that circle to the right or make it large. it is totally subjective.

The size of the circle is random, but it is placed correctly, those are the cities with the world highest living standards and below average costs of living compared to the standard they offer. Best value deals, in the regard of a company that has to send employees abroad.

The blue line simply indicates the trend that nicer cities tend to be more expensive to live in. If you divide the graph into quadrants, the upper left is the dreamzone cheap but nice cites, the upper right is the gold-plated nice but expensive cities, the bottom left is unpleasant nasty but cheap cities and the bottom right is the nightmare expensive but crap cities.

The blue line is how high the costs of living are on average compared to the quality of life the respective city is offering. The grey area is the standard deviation, everything within is "normal" or expected deviation from the average. If a city is above the blue and grey lines, it means it offers a comparatively high standard of living compared to the costs, if a city is below the blue and grey, it offers a comparatively low standard for the money it takes to live there.
The whole study was done not for people, but for fictional multinational companies, so they can compare how much living quality a possible location would offer and how much they'd need their employees for in costs of living.
