I saw a video on youtube of a training expert showing how to shoot and stab at the same time. I can't find the video. It was a Black man at an indoor range.

only if you're in an extremely enclosed environment, you have very little ammo left in the mags and no spare mags. And you happen to have a knife on hand. Even then you wouldn't use an ice pick grip, but have the knife point up or forward cinched between the gun and your left hand palm

Sorry to spoil a a 13 year old game, but the blonde guy is actually CIA and basically working undercover as the enemy, and has orders to make sure Snake succeeds in his mission. Sure, they're using live ammo, but he's probably told his team to hold back a little.
