well it is very hard since there are lots of distractions, counter procedures against anti missle systems, for example after reentry they can tumble the missle making its trajectory very unpredictable, aim also decreases but well it is a nuke, or send lots of decoys with it making it hard to hit them all

They typically won't "explode", detonation of a nuclear weapon isn't based on getting blown up, it's a pretty ordered thing and the way they detonate has to be a very ordered process. It's more complicated then you might think. Also considering the altitude they wouldn't do as much harm as you'd think either.
