Candito has about the cleanest squat form you'll find and he badly injured his back on a submaximal set with no form breakdown, plenty of people in /plg/ also get hurt on totally normal reps that are well below their limits

once you start get intermediate level you're liable to snap you're back up even with perfect form. See Mike Tuscherer, snapped his back up as well with 0 form breakdown. Layne Norton with the worst squat form ever snaps his back up monthly.

just go slow, rest enough, use proper form, be flexible, and don't lift too heavy
