Most are noticed at a young age (aggression and fortitude are sought) then given a strict military training, then a survival test before being genetically altered and given new organs.
THEN they begin their training to be Space Marines (they almost all start as scouts).
A small percentage of the children that are initially noticed ever make it to the end.

I've only read 5 or 6 of the books but someone posted an excerpt of a captain telling one of the marines under his command to quit working out so much as he was getting too big and the admech was refusing to remake his power armor to accommodate his size. So they can get bigger from working out.
