Read the sticky. Diet is more important for you than exercise if you want to just lose weight. You need to eat less calories than your TDEE. You will find info about dieting in the sticky.
If you crash and gain again then probably you have also some kind of issues with discipline, not motivation. Start tracking calories, it is very important. Since you probably do not see immediate results of dieting, because you failed in the past, I'd recommend you start lifting weights. Not only they will make you look better, feel better - you will also have a small achievement every time you leave the gym (more weight lifted).
Nobody will give you cookie cutter diet plan because 1. it takes time, 2. it is fucking retarded, just start eating healthy foods (read the sticky) and count the calories.

Empirically find your calorie maintenance level, then eat under maintenance (I'd recommend gradually lower your calorie intake approximately 100 a week) by ~500 cal. Eat clean: no junk food, no sugar in any form (soda, sweets etc). Eat a lot of protein and don't forget about fats, you need them, but get it from good sources: fish, avocado, nuts. That's almost all you need to lose weight. You just need to do it consistently for a long time. Now about lifting. I don't know how things are going where you live but here in my gym there are group classes for women (for men too but you know, men usually go to the gym) 1 hour long, 3-5 times a week should be enough for a 'toned' body and an addition to a weight loss.
Fiber and protein are great for satiety. So think, slab of meat, with a pile of veggies. Avoid empty carbs like bread bought from the store, noodles, rice, and potatoes.

Find local groups online. My city has a bicycling group that goes out and rides several nights a week. We have a running group. We have a couple of fitness groups, where it's just people meeting up in a park and doing calisthenics for an hour.

If so literally copy their diet, you’ll have to work down to it gradually because of your appetite. And by diet I don’t mean "short term weigh lose meal plan" I mean your eating habits for the rest of your life.
Cut out all the sugar and fast food and eat less. It’s that simple but if you want to get permanent results it’s needs to be your permanent eating habit.
As for exercise do a weight routine to get a decent figure, go for light jogs and swims on days you don’t go to the gym.

Buy some chicken breast and a big bag of spinach and I guarantee you’ll get full fast, but you have to simply ignore sweet tooth cravings. The best way to beat sugar cravings is start eating foot, I find oranges/mandarins, watermelon and berries (particularly blue berries) work wonders for me

When you first start out you probably will lose motivation. The key is to make working out habitual, like brushing your teeth. Eventually you'll feel discomfort if you don't work out.
Start slow, push yourself harder. You aren't going to walk into the gym and deadlift 6 plates the first day. You start small, and work your way up, pushing yourself harder each time.
When I first started riding my bike, I could barely do 8 mph. But I just kept pushing myself just a little bit harder. For the first week I just aimed at doing that 8 mph for an hour, each time I went out. The next week, I pushed myself to 10 mph for an hour. So forth and so on. Start small, but keep pushing the limits. Now I can comfortably ride 20 mph as long as I want.

So your looking for things that would keep you motivated ? Its quite simple :
1)Friends - Im talking about friends who work out like you do. If your current friends don't work out , it doesn't matter , as you would still find new friends in gym.
2)Self improvement - When you see yourself improve and get closer to your goals , you get more motivated to continue on the path of self improvement.
3)Habit - Once it becomes a habit to go the the gym every day on certain weeks , you would actually miss it if you don't do it , and would feel bad with yourself. So basically if you succeed for around a month or 2 , your likely to fully succeed.
For conclusion JUST DO IT - Go to the gym get a membership and do your training TODAY. If need be , force yourself to do it for a while(I know its hard to fight your own laziness) , until it just becomes your instinct to do it.

It's not about changing your whole diet and routine at once. That's something that ex drug addicts need to do but it's more reasonable for them.
Just make changes like cutting out soda or candy and stuff like that. Just make like one change every two weeks and keep weighing yourself.
The gym doesn't even have to be a part of it, but it's a fantastic way to speed up the process.
For a female I would recommend watching megsquats if your into that, she's by no means the best resource but she's not a bad one
