I was watching this webm and it made me think, what did this guy do wrong? How does one prevent this from happening apart from not being retarded? How bad is his form? All I can tell is he stuck his ass too far back for his center of gravity but I cant really think of a better fix

He caught it with his neck, the bar should be more on his shoulders/clavicle. He should've dropped the weight the second he realized that he caught it improperly, everything after that was retarded ego lifting.

Go do a 1rm OHP or deadlift and don't breathe during the entire thing, see how dizzy you'll get if you're actually bracing your entire body. Your muscles use all the blood and you brain shuts down to protect itself from permanent damage

He's obviously strong enough to attempt that lift, he nearly had it. He let his ass drift back and then he tried to "Drive through his heels" too much, coupled with the bar being too high on chest, he was basically falling backwards through the entire lift.
Dafuq man how long does it take you to do a deadlift? You don't breath thru the lift, you inhale at the start and exhale as you lift
IIRC if you put pressure on your clavicles it greatly reduces blood flow to the brain.

The problem was his breathing. This sometimes happens to me during OHP as well. You get really really fucking light-headed.Sometimes you can "catch yourself" and rebalance at the top. Other times you basically blank out and the weight comes crashing down back to the rack.

The brain doesn't "shut down". Blood restricting results in lowered consciousness which signals muscles to relax causing you to drop so that your head levels with your heart and receives blood again. At no point does it go on "power saving mode".
