i personally don't like that as a concept, i am only a tire kicker though.
i would rather warm up as little as possible and then let loose on the working sets. doesn't mean it doesn't work, probably allows you to accumulate more volume albeit with a sub optimal weight.

I usually throw in. 2x15 to failure on the tail end of a movement, however I may switch focus to a solid 3x4-6 at higher weight. Trying to find that sweet spot that high volume got me, but without the 2 hour time investment.

The "time on = time off" thing made me wonder what would happen if someone did a month-on-month-off system. Like, if you went to the trouble to make sure that everything was cleared out after one month (short esters, or frontloading long ones and riding it out), took a month on cruise, then did another one-month blast, then took another month on cruise, and so on.

Do we have any examples of people doing this? It's probably stupid, but I'm curious.

As I said I'm only a tire kicker but it sounds like you are doing alot of unproductive work (which you still need to recover from).
The 2x15 at the end. Why, you have done your most intensive/stimulating set (the 4-6). Those extra two sets are just more to recover from after the muscle damage has already been done.
All the fatigue from the 12-10-8 is going to take away from your 4-6 (which I think is a shitty rep range to work in for hypertrophy as well as a high-risk range after accumulating that fatigue)

the body likes long periods of consistency for muscle growth and healing. you get neither doing month on month off.

Assuming the gains were good on 5x12 my initial thought would be drop to 2x12 for 1-2 months, priorities increases in load/re-sensitise to volume. after the 1-2 months incrementally increase back up to 5x12 with the now heavier weights

How heavy do you go? Past two months I've been doing the dumbbell lateral raises where you hold onto something and lean way to the side you're working but because of that I've gone significantly down in weight for it.

I use between 12.5 - 25lbs for lateral raises depending on if I’m hitting shoulders or chest that day. You can feel it in your outside delt (forget what it’s actually called) when you hit it right.

I’ve experimented a lot with lateral raises. Something like 30s/20/10x10 for 3-4 sets works well. However I prefer 20-25s x15 for 4-5 sets. Drops when needed.
