Metabolic syndrome is a disease. We actually already understand a lot about it, but it's not common knowledge. It's not as simple as dieting and exercise. Obese people often have systemic inflammation and organ damage that can be difficult to heal.

In a science sense, metabolism refers to the overall chemical reaction chain in the entire body regarding energy processing, conversion, and use, and is entirely too encompassing for even moderately educated people to understand without classes in biochemistry and macronutrient metabolism

Physicianfag here. Metabolic syndrome is a term that describes a constellation of conditions (HPT DM Dyslipidemia visceral adiposity...) that are the direct result of obesity. It is not an explanation as to why obese people get these diseases but merely an observation that they commonly occour together in groups.
