Focusing on the largest muscles in the body (legs) should theoretically help you add the most muscle mass which increases your metabolism and helps to burn more fat.

For one, obese people are terribly out of shape, and heavy cardio typically tends to spike appetite and compensatory eating.
Second, for a fat person out of shape, "heavy' cardio for them isn't going to be all that much in terms of calories, especially relative to weight needing to be lost. Also they may go to snap city.
Third, a routine too extreme will deter compliance.
Fourth, heavy cardio only will result in about 1/3 of weight lost being muscle. That's not great for body comp.
A sustainable program with a substantial deficit, some lifting and a gradually escalating low-impact cardio is going to make compliance and success far more likely

legs and back will have the most easy gains in terms of sheer lbs of muscle, shit like abs or other cosmetic muscle groups barely help

Former obese struggler here. Get them running, doing pushups and sit ups and eating lower calorie high protein meals. Train nothing else until they lose at least half of what they need to. So many times i would just start weight training without losing enough fat first, see no difference and give up. Clear the slate until they can actually see body and not just fat.
