My best bench was with an axle bar that was heavier than that and I'm fine.
If anything, the smooth, thicker axle bar is even more prone to slipping than a conventional olympic bar.
Maybe he missed his cue about bending the bar so his grip wasn't where it was supposed to be.

Like I said, finding the right hand position is not something a newbie or even intermediate should do.
They'd be too inconsistent, even if they're only off by a few millimeters.
Though you could cheat and leave some markings on the bar itself for reference but then you'd be a huge bitch.
Half the fun is having one side a tiny bit heavier but finishing the set anyways.

for beginner and intermediate lifters, the hand placement is not as big of a concern. they could swich dozens of times till they find the grip that works best for their levers and bench style. even top benchers experiment with different grip widths. the only peice that matters is balance, which you mention, but there should be some reference point to start from on the bar. if theres not, its not a very good axel.
You dont need a safety squat bar to squat. learn lowbar and be done with it, theres no reason you shouldn't be able to do that. theres not really an excuse for that either.

How long did it take you to do that from 4pl8s?
ive beenched 500 for the first time when i was 19 summer between school, so about a year i think. thats awhile ago though so cant be certain how long. Various life choices, eg. going to school at a military college, (usma) hasnt kept strength levels consistent, and i started training oly so i stopped caring about bench for a while (3 years). It went from 450ish at 265 to 540 at 300ish in abit more than half of this year, but i cant really use that as a bench mark for you since theres a base there that just needes to be trained back to bench. im 6'4 so the 308 powerlifting class is where ill be sitting. biggest concern is deadlift and squat right now, because their lagging behind uspa international standards right now. looking for 800 diddy, but going is slow.
get a slingshot for overload work and do dedicated back work too. Check how Josh Bryant has maddox and Strickland train backband and try that if your not already
