It seems there's a massive drop-off point somewhere. Their top-500 per capita ranking is not good. So your options as an American seem to be to work yourself to the bone to be in one of the best unis in the world or settle on a less than mediocre one? Have I understood this right?

no, not at all. I think you are conflating a lot of people going to shitty universities with there being a lack of access to good universities, this isn't true. Many GI bill recipients chose to go to for-profit colleges like ITT Tech, even if better universities in terms of education would have taken them.
there are just as many opportunities at any large state university as there are at the universities listed above. I go to Berkeley, my friends at ASU from high school are on the same paths in their careers as I am. At the end of the day, it seems like the only substantial difference is prestige.
Of course, there are differences in class offerings, possibly more talented faculty, smaller student to faculty ratio, etc, etc...but do these things necessarily result in a better educational experience for an undergrad... except for range of class offerings, I am not so sure.
if you intend to go to professional school (MD, JD) after graduation, going to a state school might even be a better option... better to be a big fish in a small pond, you know...
at the end of the day, any university with "very high" research output as listed here carnegieclassifications.iu.edu
