how is the medical situation in bulgaria? i heard that all young doctors head for west and central europe because they get paid like shit in bulgaria and at the same time lots of poor german pensioneers move to bulgaria because there they don't have to live in poverty with their small pension

That's pretty much correct. People do study over here, and then usually flee abroad in search of higher pay --- who'd work for 1/7th of what people in their sphere get in say, Germany or France? The EU is making the migration easier, too, so there's little incentive for them to stay.

I don't think I've really felt any problems like that, but then again, I don't go to doctors much. I haven't made full use of what's available, as to say.
I've never had anything more serious than an ear infection happen to me in recent times so I don't think I can respond properly.

not the guy you replied to, but this is literally the first thing I can think of, as it is the most obvious and immediate way the brain drain affects me.
I have 2 friends who studied medicine, both of them work in Germany now.
Meanwhile there is a very serious deficit of medical personnel here and the quality of care is awful, even in the capital all the doctors are overworked and grumpy and generally don't give a shit about you.

doesn't this whole system give an incentive to the bulgarian state to educate shitty doctors? Right now they pay for the education and get shafted royally when they leave the country, wouldn't it be better from their viewpoint to produce shitty doctors that have no chance of employment in the 'rich' countries?

The state has indeed lost many billions in educating people who then migrate, our prime minister has spoken about this - he is generally a pumpkin, but he's right on this one.

From what my friends have told me, the medical education in Bulgaria is worse than in Germany but is still good enough as they don't really have troubles working and specializing there and bringing themselves up to speed as they go.
